Through several initiatives, Nasson Health Care noticed that their clinical quality metric outcomes were lower than other health centers in Maine. In working with York County Community Action Corporation (YCCAC), they were able to see that social services weren’t effectively integrated as part of care. To address this, Nasson and YCCAC worked to improve data collection and referral processes.
