What is PRAPARE?
Protocol for Responding to & Assessing Patients’ Assets, Risks & Experience
(PRAPARE) is a national standardized patient risk assessment protocol designed to engage patients in assessing & addressing social determinants of health (SDOH). PRAPARE is evidence-based, designed through stakeholder engagement, paired with an Implementation and Action Toolkit, and standardized across ICD-10, LOINC, and SNOMED.

*Note: The PRAPARE screening tool gathers information on “personal characteristics,” including race, ethnicity, and language preference. These characteristics are not inherently connected to poor health outcomes, however, the social and economic conditions affecting patients’ health are deeply rooted in systemic racism, discrimination and inequality. With data, we can demonstrate the disproportionate distribution of social risks within our communities, and use this information to more effectively work upstream to create healthier, more equitable systems and communities.
