PRAPARE® Implementation and Action Toolkit

The PRAPARE® Implementation and Action Toolkit is a comprehensive resource designed to equip users with practical tools, best practices, and lessons learned to support effective implementation, data collection, and responses to non-clinical factors of health. Built on the experiences of community health centers that have successfully championed PRAPARE®, the Toolkit offers guidance for addressing non-clinical factors that impact patient health.

The PRAPARE® Implementation and Action Toolkit is designed to support health centers and other organizations in effectively implementing the PRAPARE® tool to collect and act on data related to patients’ health related needs and circumstances.
 
Whether you are just beginning your PRAPARE® journey or looking to enhance your current approach, the Toolkit offers step-by-step guidance to help teams prepare, collect, assess, and respond to non-clinical factors of health data.

PRAPARE® Readiness Assessment

The PRAPARE® Readiness Assessment Tool helps organizations evaluate their preparedness to implement PRAPARE® effectively. It guides teams through key operational, technical, and strategic considerations to support successful planning, integration, and sustainability of non-clinical factors of health data collection.
 
This tool is ideal for both new and experienced users looking to strengthen their PRAPARE® implementation efforts. It helps identify strengths and gaps across four critical areas:
