Nālani Tarrant

Director, Whole Person Health

Nālani Tarrant, MPH, PMP, serves as the Director of Whole Person Health at the National Association of Community Health Centers (NACHC), where her leadership is instrumental in advancing collaborative initiatives with Federally Qualified Health Centers (FQHCs) to strengthen non-clinical factors of health. Through her work, Nālani supports health centers in designing and implementing approaches that improve care for all and foster thriving communities.
Her portfolio includes program development, implementation, and comprehensive evaluation with a dedicated focus on the Protocol for Responding to and Assessing Patients’ Assets, Risks, and Experiences (PRAPARE®). Under her guidance, PRAPARE® has evolved into a trusted framework that helps health centers better understand and respond to the factors influencing individuals’ and families’ well-being.
Nālani holds a Bachelor’s degree in Behavioral Science from Drew University, a Master of Public Health in Epidemiology from The George Washington University, and earned her Project Management Professional (PMP) credential in 2017.

Cloé Destinoble

Project Manager, Health and Well-Being

Cloé Destinoble, MPH, serves as the Project Manager, Health and Well-Being at the National Association of Community Health Centers. She equips health centers with data-driven strategies for documenting and addressing patient non-clinical factors of health. This work involves developing partnerships and building health center capacity to collect and utilize standardized vital conditions data. Cloé oversees a national portfolio of community health initiatives, driving project management to improve health outcomes, address non-clinical factors of health, and improve maternal and child health—ultimately achieving significant and measurable positive impacts within the safety net healthcare system.Cloé began her career focused on optimizing provider workflow and enhancing efficiency. She transitioned into public health roles, where she leveraged communication skills to disseminate improving access to care for all people public health information through written materials (including digital health messaging) and oral presentations, aiding in disease transmission prevention.Subsequently, Cloé has managed projects that include the development and distribution of relevant resources, the provision of continuing education and technical assistance, and the administration of programs supporting frontline clinical care for underserved individuals. Her experience also includes serving as an HIV training program leader for health departments and contributing as a Biological Scientist in epidemiology. Cloé holds a Master of Public Health in Health Promotion and Disease Prevention with a concentration in Maternal and Child Health from Florida International University, and a Bachelor of Arts in Psychology from Florida Atlantic University.

Cydnee Parsley

Deputy Director, Whole Person Health

Cydnee Parsley, MPH, is the Deputy Director of Whole Person Health at the National Association of Community Health Centers. In this role, she supports health centers in identifying patient-level non-clinical needs and leveraging data to drive upstream community initiatives that promote health access. Cydnee began her career with Special Olympics Florida, collaborating with community health centers, healthcare providers, and universities to implement health and wellness programs for individuals with intellectual and developmental disabilities (IDD). She holds a Master of Public Health degree from the George Washington University, with a concentration in health policy and communications. Her expertise encompasses the development, implementation, and evaluation of disability-centered health programs to support healthcare providers and consumers.

Rachel Ram

Program Manager, Whole Person Health

Rachel Ram, MPH, serves as the Program Manager of Whole Person Health at the National Association of Community Health Centers. In this role, she supports community health centers across the country in collecting and applying data to strengthen partnerships and improve community well-being.Rachel brings a multidisciplinary background spanning nonprofit program management, Medicaid finance consulting, health education, and community-based research. She most recently worked at Sellers Dorsey, supporting initiatives to strengthen the Medicaid workforce and programs through innovative funding strategies. Previously, she contributed to chronic disease prevention and implementation research at the University of North Carolina and led tobacco cessation, asthma, and lung health programs at the American Lung Association.She holds a Master of Public Health in Health Policy from the UNC Gillings School of Global Public Health and a Bachelor of Science in Health Education and Behavior from the University of Florida.

Raymonde Uy

Physician Informaticist

Raymonde Uy, MD, MBA, ACHIP is a Physician Informaticist at the National Association of Community Health Centers.  Dr. Raymonde Uy supports the clinical informatics needs of projects funded by the CDC and others on nationwide community health center health information technology and population health concerns. Prior to joining NACHC, Dr. Uy serves as a clinical informatics consultant and visiting professor in Clinical Informatics with academic and industry stakeholders in both the United States and the Philippines. Dr. Uy also works as a scientific reviewer for both the American Medical Informatics Association (AMIA) & the International Medical Informatics Association and is an member of The Office of the National Coordinator for Health Information Technology’s Health Information Technology Advisory Committee and Adopted Standards Task Force. Dr. Uy earned a dual MD/MBA degree: Doctor of Medicine and Master of Business Administration, and completed his postdoctoral fellowship in Biomedical Informatics at the U.S. National Institutes of Health. Dr. Uy is a diplomate of the inaugural AMIA Certified Health Informatics Professional class, and is the commissioner of the health informatics certification committee.
