Shachar Gazit-Rosenthal, MPH, serves as the Specialist, Vital Conditions at the National Association of Community Health Centers (NACHC), where she supports whole-person health initiatives focused on strengthening how health centers identify, document, and respond to non-clinical factors of health. In this role, she contributes to fairness-centered, data-informed efforts that support the implementation and use of PRAPARE and related tools to improve care delivery and inform upstream, community-driven strategies. Shachar holds a Master of Public Health with a concentration in Behavioral and Community Health from the University of Maryland, where her training emphasized mental health support, health for all, and family health. Her academic and professional work reflects a strong commitment to community engagement, whole person care, health literacy, and translating research into actionable, evidence-based practice. Professionally, Shachar has experience in community-based research, program coordination, and public health project management, most recently serving as Project Manager for the Water Emergency Team (WET) research project at the University of Maryland. Across her work, she has focused on advancing health fairness through collaborative partnerships, responsible data use, and programs that support communities in addressing the conditions that shape health and well-being.
